Escalator Injuriesâ?¦are We Putting our Children at Risk? Does Shoe Gear Matter?
The rising popularity of flip flop sandals and Crocs shoes, as well as a few well-publicized incidents, has increased awareness of escalator injuries in children in the United States. These two types of shoe gear have been lumped together as increasing risks, but is this a truly fair assessment? What is the truth about the risks involving our children and escalators? Is awareness enough to protect our children? Do the shoes our children wear on escalators matter? Should we all be taking the stairs or the elevator? As a physician and mother of three small girls, I am concerned about escalators and our children.
Letâ??s look at the numbers. In the United States there are an estimated 35,000 escalators with approximately 245 million riders daily. The Consumer Product Safety Commission estimates that about 10,000 people go to the emergency room every year after accidents on escalator and 20% of these injuries involve entrapment of feet, hands, or shoes. Thatâ??s approximately 2,000 incidents involving some type of shoe. In comparison, very few incidents have been reported involving Crocs shoes. That means, given the number of emergency room visits per year for such entrapments, which usually involve softer shoes or bare feet, the fact that a relative few incidents have been reported in Crocs shoes is basically insignificant. Also given the number of kids wearing Crocs today, it only stands to reason that eventually an entrapment would occur, as Crocs are not immune to being grabbed by escalators, any more than flip flops, jelly shoes, sandals or soft running shoes.
What does put our children at risk? The fact that escalators are dangerous to small children has been well known for years. In fact, in 1997, in the Journal of the American Academy of Pediatrics, it was reported by a group of physicians from the Department of Pediatrics, New York University School of Medicine and the Pediatric Emergency Service, â??Children are at risk for sustaining severe injuries on escalators. Young age, inadequate adult supervision, improper activity while riding on the escalator, and escalator-related mechanical problems all increase the risk of injury. Public and parent education directed toward escalator safety issues may help to reduce escalator-related injuries in children.â? The largest incidence of injury was reported to be involving children between 2 and 4 years old (50%) with the average age being 6 years old. It should also be noted that 60% of children fell prior to their injury.
The action of stepping off the escalator is associated with an increased risk of injury. Young children may remain standing on the escalator and allow their feet to slide off at the end, instead of actually stepping off. The small size of a child’s foot may increase the risk of it slipping into the gap where the last step slides into the comb plate. While stepping off an escalator may seem like a simple and natural task to an adult, the developmental level of young children limits their ability to both anticipate and coordinate this action. In addition to feet becoming entrapped during the process of stepping off, children’s small extremities may become lodged between two steps or between a step and the side-rail.
We can continue to ride escalators with our children if we follow some simple safety tips from the Consumer Product Safety Commission:
1. Loose shoelaces, drawstrings, scarves and mittens can become entrapped. Make sure a childâ??s clothing does not put them at risk.
2. Always hold childrenâ??s hands, just like crossing the street!
3. Do not permit children to sit or play on the steps.
4. Do not carry children in strollers, walkers, or carts. Use the elevator.
5. Always face forward and hold the handrail. If you fall, you put your child at increased risk!
6. Avoid the edges of the steps where entrapment can occur.
7. Always pay attention and alert while riding with your children, note where the emergency shut off is.
Prevention of escalator-related injuries is the key and efforts should be focused in two directions. Safety education for parents should include and give specific guidance regarding injury prevention about riding on escalators. Increased parental supervision should be encouraged, such as hand-holding or even carrying of young children while riding on and especially while stepping off escalators. Children should be taught not to run, play, or sit while riding on an escalator. Children should face forward and hold the handrails.
The bottom line about escalator injuries is that no matter what shoes a child is wearing, if the parents are not supervising them closely, injuries can occur. There is no specific correlation between shoes and injuries; they occur even in the best of circumstances usually due to inattention and childrenâ??s unsafe behavior. Comparing Negative Casting Techniques: Foam versus Plaster of Paris Richard Berenter, DPM, FACFAOM Introduction: This study was undertaken to determine whether there was any difference in the clinical outcomes related to the type of negative casting technique utilized in the manufacture of functional foot orthoses. Those practitioners who favor foam casting blocks argue that the technique is cleaner, faster, more cost effective and just as reliable a method to produce functional foot orthoses versus the plaster of Paris technique. On the other hand, a number of practitioners have argued that the foam block technique is inferior because the foam is incapable of capturing the shape of the foot with the subtalar joint in neutral position and the midtarsal joint maximally pronated thereby leading to an inferior foot orthosis, which will be less effective at reducing patient symptoms. Materials and Methods: A total of 38 patients were enrolled in the study. All of the patients presented with lower extremity symptoms associated with abnormal lower extremity function as determined by gait evaluation. At the time of the initial visit, each patient signed a consent form and completed the top portion of the data sheet which included both personal information and the amount of pain in each extremity (patients were asked to circle the amount of pain on a scale from 0-10 with 0 being no pain and 10 being the worse pain ever felt). Upon completion of all paper work, both feet of each patient were casted via the semi-weight bearing foam block technique and by the non-weight bearing supine plaster of Paris method. Both sets of casts were sent to a professional orthotic laboratory with a prescription filled out for an orthotic shell with a medium amount of arch fill, average heel cup depth, normal orthotic width (to the lateral border of the 5th metatarsal and bisection of the 1st metatarsal shaft) and a thickness of polypropylene which would behave in a semi-rigid behavior for the patient’s stated weight. A laboratory technician was instructed to randomly select one of the two pairs of negative casts and keep track of which casts were used without the knowledge of the principal investigator. In this way, a double blind study was established since neither the principal investigator nor the patient knew which casts were used to construct the foot orthotics. Approximately 2-3 weeks following casting, the patient was dispensed a pair of functional foot orthoses and asked to walk around for a minimum of 10 minutes to gauge the comfort level of the orthotics. Each participant was asked to use one of 4 descriptive terms (very comfortable, comfortable, slightly uncomfortable or very uncomfortable) to describe the comfort level of 5 different regions on each foot orthosis corresponding to the heel region, medial arch, lateral arch, middle of the orthosis and distal edge. Patients were then sent home with standardized break-in instructions for the functional foot orthoses and returned to the clinic at intervals of 2 weeks and 4 weeks post-orthotic dispensal. At each follow-up visit, patients were asked to fill out a data sheet gauging the level of symptoms and comfort level of the orthoses. The data was then compiled and saved in a spread sheet format and upon completion of the study, the laboratory technician was contacted in order to identify which patients belonged to which study group, the foam box or plaster of Paris casting technique. Results: The data was compiled and the two study groups separated by casting technique. An independent investigator (non-podiatrist) was contacted and asked to analyze the data to answer the following questions: 1. Does the negative casting technique (foam vs. plaster) make a difference in the ability of the orthotic device to reduce symptoms? 2. Does the negative casting technique (foam vs. plaster) make a difference in how comfortable the orthotic device feels to the patient? The data was analyzed in a variety of methods such as the mean reduction of pain, Fischer exact test and Chi-square with T-tests. A simple comparison of the average reduction of pain after four weeks of orthotic therapy indicates that the plaster of Paris orthoses achieved a mean decrease of 82.43% of pain versus 61.14% reduction in pain with foam box cast orthoses, with a level of significance p

Roller Derby Girls Helping the Community
In spite of a strong wind that threatened to carry our check-in tents away, we helped the YWCA staff and volunteers set up the shoe station, where the male participants would be able to choose women’s shoes with the right color, style, and fit for their walk. From SJSU athletes to San Jose firefighters, policemen and EMTs, these walkers had SVRG girls help them select mules, pumps, and stilettos in their sizes. We also outfitted them with event tees, in case their flashy heels didn’t give them away as YWCA supporters.
When it came time for the walk to begin, we packed up our skates and moved out to the half-mile point. Our YWCA coordinators paired us perfectly with our next role: offering water and band-aids to the participants. After all, who knows more about blisters than rollergirls? We handed out water to the walkers, some striding and some stumbling, and cheered them on as they passed us.
Once the crowd passed our station, we packed up and followed the high-heeled walkers to the California Theater in downtown San José, where participants and sponsors were rewarded with prizes for fundraising and refreshments.
Overall it was a great afternoon—a fun and lighthearted way to raise community awareness for some very serious issues. Next year, we hope to cheer on our own SVRG male referees as they complete their first mile in our shoes.

Silicon Valley Roller Girls (SVRG) is an all-female flat track roller derby league established in 2007 and represents what is known as Silicon Valley, California. SVRG aims to bring an additional exciting sport to the communities of Silicon Valley and allows women from all walks of life a chance to show their athleticism, intelligence, individuality, inner beauty and strength by participating in the subculture phenomenon of roller derby. SVRG is entirely skater-owned and run. Its dedicated members represent Silicon Valley’s mothers, professionals and students. SVRG encourages competition between other leagues and strives to represent Silicon Valley with sportsmanship and excitement.
2008 marks SVRG’s first season of bouting against other leagues. This year, in addition to increasing our numbers and creating 2 or 3 distinct teams of our own, we hope to achieve our accreditation from WFTDA, the women’s flat track derby association.
